These Terms and Conditions govern your use of BloomSmile.com and your enrollment in the Bloom Smile dental savings membership program. By using this website, enrolling in a membership, or submitting payment, you agree to these Terms.

Important: Bloom Smile is a dental savings membership program, not dental insurance. It is not a health plan, insurance policy, qualified health benefit, Medicare plan, or substitute for dental insurance.

1. What Bloom Smile Is

Bloom Smile is a dental savings membership program that provides included preventive dental services and discounts on eligible dental services when received from participating dental providers that accept the Bloom Smile membership program. Members are responsible for the cost of any services not specifically included in their membership benefits.

2. Membership Plans and Fees

Bloom Smile membership options may include:

  • Bloom Basic: $399 per year or $49 per month
  • Bloom Plus: $498 per year or $59 per month
  • Confidence Booster Add-On: $99 per year or $10 per month

Each membership includes, per 12-month membership year, two cleanings, two exams, routine X-rays, and up to 20% off other eligible dental services. Included preventive services assume the absence of periodontal disease. Periodontal treatment, deep cleanings, and other non-routine services are discounted, not included.

3. Membership Term, Billing, and Auto-Pay

Membership begins on the date of enrollment and payment and continues for a 12-month membership term. Members may select annual payment or monthly auto-pay, when available.

By enrolling, you authorize Bloom Smile and/or its payment processor to charge your selected payment method for the membership plan you choose. Monthly memberships renew each month and annual memberships renew each year unless cancelled according to these Terms.

A valid payment method must remain on file. If a payment fails, membership benefits and discounts may be paused until the account is brought current.

4. Included Services and Discounts

Membership benefits are available only through participating dental providers that accept the Bloom Smile membership program.

Included services are limited to the specific services listed in your selected membership plan. Services not listed as included remain the member’s financial responsibility, less any applicable Bloom Smile membership discount.

Membership discounts cannot be combined with dental insurance, government benefits, other discount plans, coupons, or promotions unless expressly permitted by the participating provider.

5. ProGlow Whitening Kit Promotional Gift

The complimentary ProGlow Professional Teeth Whitening Kit is a promotional membership benefit offered by Bloom Smile, subject to availability, and has no cash value.

  • Annual Members: The kit may be provided after enrollment and full payment of the annual membership fee.
  • Monthly Members: The kit may be provided after three successful monthly membership payments have been received.

If a membership is cancelled, reversed, charged back, or becomes delinquent before eligibility requirements have been satisfied, the promotional kit will not be provided.

If a member receives the kit and subsequently cancels membership before completing the initial membership term, Bloom Smile may consider the value of the promotional gift when determining any refund eligibility.

6. Cancellation and Refunds

Monthly memberships may be cancelled with thirty (30) days written notice. Payments previously collected are non-refundable, and cancellation will prevent future recurring charges after the notice period has expired.

Annual memberships may be cancelled within three (3) days of enrollment for a full refund only if no membership benefits, services, discounts, or promotional gifts have been used or received.

After this period, any refund eligibility may be reduced by the value of benefits used, discounts received, and promotional items provided through the membership program.

No refund will be issued when the value of benefits received equals or exceeds the membership fees paid.

7. Member Responsibilities

By enrolling, you agree to:

  • Provide accurate and current enrollment and billing information
  • Keep a valid payment method on file
  • Use membership benefits only for the enrolled member
  • Notify Bloom Smile of billing or account changes
  • Follow all applicable provider policies, appointment policies, and payment policies

8. Non-Transferability

Bloom Smile memberships are non-transferable and apply only to the named member or enrolled household members, when family pricing applies.

9. Family Pricing

Family pricing may be available for members enrolled in the same household. Current family discounts may include 5% off for two members, 10% off for three members, and 15% off for four or more members. Family pricing may change at renewal with reasonable notice.

10. No Medical or Dental Advice

Information on BloomSmile.com is provided for general informational purposes only and does not replace professional dental advice, diagnosis, or treatment. Dental care decisions should be made directly with a licensed dental professional.

11. Dental Services

Dental services associated with the Bloom Smile membership program are performed by independently owned and operated participating dental providers.

Bloom Smile does not practice dentistry, provide dental treatment, diagnose conditions, or make clinical recommendations. All treatment decisions, diagnoses, fees, and patient care are determined solely by the participating dental provider.

Participation in the Bloom Smile membership program does not guarantee treatment eligibility, treatment outcomes, provider availability, or appointment availability.

12. SMS and Email Communications

By enrolling or submitting your contact information, you agree to receive membership-related emails, calls, and text messages from Bloom Smile. Message and data rates may apply. You may opt out of marketing text messages by replying STOP.

13. Website Use

You agree to use BloomSmile.com only for lawful purposes. You may not misuse the website, interfere with its operation, attempt unauthorized access, or use the site in a way that violates applicable law.

14. Intellectual Property

All website content, logos, graphics, copy, images, designs, and branding are owned by Bloom Smile or its licensors and may not be copied, reproduced, or used without written permission.

15. Third-Party Links and Platforms

BloomSmile.com may link to third-party websites, payment processors, patient portals, participating provider websites, or ecommerce stores, including BloomSmile.store. Bloom Smile is not responsible for the content, policies, or practices of third-party websites.

16. Limitation of Liability

To the fullest extent permitted by law, Bloom Smile, its affiliates, officers, employees, participating providers, licensors, and partners shall not be liable for any indirect, incidental, consequential, special, exemplary, or punitive damages arising from use of the website, membership program, or services provided by participating providers.

Any disputes relating to dental treatment, diagnoses, clinical recommendations, fees, or patient outcomes must be addressed directly with the treating provider.

Your sole remedy for dissatisfaction with the membership program is cancellation according to these Terms and any refund eligibility described above.

17. Indemnification

You agree to indemnify and hold harmless Bloom Smile, its affiliates, employees, officers, participating providers, licensors, and partners from any claims, damages, liabilities, costs, or expenses arising from your misuse of the website, violation of these Terms, misuse of membership benefits, failure to pay membership fees, or failure to comply with applicable laws.

18. Governing Law

These Terms are governed by the laws of the State of California, without regard to conflict of law principles. Any disputes shall be handled in accordance with applicable California law.

19. Changes to These Terms

Bloom Smile may update these Terms at any time. Updates will be posted on this page with a revised effective date. Continued use of the website or continued participation in the membership program after changes are posted means you accept the updated Terms.
